The Athletic Department would like to thank you and your son/daughter for submitting the required paperwork in a timely manner. The NMAA requires every student athlete to have a new physical and concussion course certificate every year.

PLEASE NOTE: Creating an account with RankOne allows you to check on your child’s status and see comments on why a form has been denied. You will not be able to check on your child’s status without an account.

Required Online Paperwork

Medical Eligibility Form
Take the entire physical packet with you to your appointment. You will ONLY UPLOAD the last (Medical Eligibility Form) page. Please upload a clear picture or pdf copy and keep the rest for your records.
The “Medical Eligibility Form” MUST be dated after April 1 st . It MUST have your child’s name at the top of the page. It MUST be signed by your physician.

Emergency Information / Medical Insurance Form
* Please fill out the form and upload front and back pictures or pdf copy of your current insurance cards.
Your child MUST have insurance or their form will get denied and they will be unable to practice or compete. School insurance can be selected on this form.

NFHS Concussion for Students Form
The NMAA requires your student-athlete to complete the following course: https://nfhslearn.com/courses/concussion-for-students
NOTE: Concussion for Students NOT Concussion in Sports
A clear screenshot, picture, or pdf of the entire completed certificate will be accepted. The certificate MUST be in your child’s name and be current (new every year).
